The marathon starts at the statue of Alexander the Great in Pella and finishes on Thessaloniki's seafront at the Alexander statue on Leoforos Megalou Alexandrou.
The organiser starts the race from the statue of Alexander the Great in Pella, the birthplace of Alexander and capital of ancient Macedonia. The finish is on the Thessaloniki seafront, on Leoforos Megalou Alexandrou at the height of the Alexander statue.

The marathon page calls the course low in difficulty. Its main feature is small variations in height; the others are a steady direction to the east, long straight stretches of road and few inhabited areas on the way up to 30 km. The organiser says the course is ideal for records and big performances.

After the second kilometre the runners join the Edessa to Thessaloniki road for the next 8 km to Chalkidona, which they enter at 8 km. At 13 km the road crosses the Axios river on a bridge and at 28 km the Gallikos river. The 30 km mark is at Ionia (Diavata) and the 35 km mark is in the centre of Kordelio, on Monastiriou street. At 38 km the course turns right on Giannitson street, the 40 km mark is at the Thessaloniki law courts, and 41 and 42 km are on Leoforos Nikis before the finish on Leoforos Megalou Alexandrou.
