Course elevation profile, 8× vertical exaggeration Elevation along the course from km 0 to km 21. The trace runs between 3 and 69 metres, a range of 66 metres, drawn at 8 times vertical exaggeration so the shape is legible. Smoothed over 1 km before this profile was derived. An unsmoothed urban surface carries the buildings and would assert a climb that is not there. The dashed stretches are bridge decks: Ponte 25 de Abril, km 0 to 3.3; (unnamed span), km 5.8 to 5.9. A bare-earth model reads the ground or the water beneath a bridge, not the deck the runner is on, so those runs are interpolated between the ground either side rather than measured, and they under-report the real climb across a span.

The 2026 course started on the 25 de Abril Bridge deck, about 69 m above the river, and ran north across it. By km 7 the road was down to about 5 m, and from there to the finish in front of the Centro Cultural de Belém it stayed between 3 and 6 m.

The course climbs 13.2 m in total, so nearly all the height on this course is given away in the first 7 km.
