Course elevation profile, 8× vertical exaggeration Elevation along the course from km 0 to km 42. The trace runs between 122 and 179 metres, a range of 57 metres, drawn at 8 times vertical exaggeration so the shape is legible. Smoothed over 1 km before this profile was derived. An unsmoothed urban surface carries the buildings and would assert a climb that is not there. The dashed stretches are bridge decks: Via Volturno, km 13.1 to 13.3; Via Volturno, km 22.5 to 22.7. A bare-earth model reads the ground or the water beneath a bridge, not the deck the runner is on, so those runs are interpolated between the ground either side rather than measured, and they under-report the real climb across a span.

The road runs from 122.4 m, at 34.8 km, to 179.0 m, at 8.0 km: 56.7 m from the lowest point to the highest. The marathon climbs 90.1 m in total, so a watch on the day will count more small rises than this from overpasses and underpasses.

The organiser gives the same shape in its own words: start at 156 m above sea level, finish at 152 m, a drop of only 4 m, and the highest point around the 8th kilometre at 181 m.

The organiser describes racing in Brescia as running through the centuries: the remains of Roman Brixia in the Archaeological Park with the Capitolium, then the passage under the Arco del Broletto that opens onto the old centre, Piazza Paolo VI with the Duomo Vecchio and the Duomo Nuovo, and the finish in Piazza della Loggia. It calls the course fast and fluid, with the smallest possible climbs.
