Course elevation profile, 8× vertical exaggeration Elevation along the course from km 0 to km 21. The trace runs between 3 and 11 metres, a range of 8 metres, drawn at 8 times vertical exaggeration so the shape is legible. Smoothed over 1 km before this profile was derived. An unsmoothed urban surface carries the buildings and would assert a climb that is not there. The dashed stretches are bridge decks: Seaport Boulevard, km 0.6 to 0.9; Longfellow Bridge, km 5.2 to 5.8; First Street, km 6 to 6.1; Memorial Drive, km 7.1 to 7.5; Memorial Drive, km 10 to 10.2; Memorial Drive, km 14.5 to 14.7; Longfellow Bridge, km 17.3 to 18; Seaport Boulevard, km 20.2 to 20.5. A bare-earth model reads the ground or the water beneath a bridge, not the deck the runner is on, so those runs are interpolated between the ground either side rather than measured, and they under-report the real climb across a span.

The organiser advises everyone to watch the signs on Cambridge Street before the split so they are on the correct side of the road. Both courses tour downtown Boston, taking in the Seaport District, the Old State House, Government Center, Faneuil Hall, Boston Common, the Theatre District and Post Office Square. The half marathon adds the Charles River by MIT and Harvard. The course has 7 aid stations in the half marathon, roughly every 1.5 to 2 miles.

Security screens everyone entering the start and finish area, and the organiser asks runners to arrive by 6:00. Runners self-select one of three corrals by pace: under 9:00 a mile, 9:00 to 11:00, and 11:00 or slower with walkers. Police and medical support leave the course after 3 hours, and a sag bus at mile 11 sweeps anyone over the cut-off. Runners slower than 13:44 a mile are encouraged to take the 5 mile race instead.
